About CINQCARE
CINQCARE was born different. CINQCARE is a provider-led network of primary care providers focused on keeping its members in control of their practice while navigating the complexities of change in the industry. We’ve established a successful track record of supporting our network of predominantly Black and Brown primary care providers. Our Physicians, practitioners, caregivers, and team members are committed every day to improving the health and well-being of those who need care the most, with a deep commitment to Black and Brown communities.
CINQCARE’s promise is to deliver health and care where our members (who we call family) live and we are committed to the success of our provider partners and ensuring their autonomy in a value-based care environment.
